I understand that no COVID-19 test is 100% accurate.

I am aware travel restrictions differ by state/country and can change daily on very short notice due to fast-changing government mandates and restrictions.

I will be responsible for performing my own due diligence to verify all other travel arrangements and requirements are met for travel to the country of destination.

I agree that Genetrack, their respective mandatories and employees will not be held responsible if I am denied boarding of my flight, or denied entry into my connecting or final destination as a result of the sample collected, testing done, or test result certificate provided by Genetrack Biolabs Inc.
